Tournament Forms

 

 

On Field Check-in    (Arrive one hour before your first scheduled game to check your team in.)

Check-In Requirements:

1. A completed Team Roster as provided for on the Pats Cup Tournament website.

2. 2012 - 2013 player identification cards and medical release forms. All teams must be affiliated with FIFA and/or USSF and players and coaches must have laminated cards. Any team member may do check-in for your team.

3. Unlimited loan players will be permitted with a maximum roster size for U9 & U10 (14 players), U11-U15 (18 players) and U16 thru U19 (22 players).      

4. You will be asked to sign a Tournament Credential/Liability Waiver Form at Check-in.

Special Notes:

1. Teams will register and play in age brackets for 2012 - 2013 season.

2. U9 & U10 ages will play 8v8 with no exceptions.

3. There will be no additions to the team roster after the team registers.

4. All coaches have complete responsibility for the conduct of their players, bench, friends, and spectators at all times.
